Please fill out the Nursing Application form completely. You must fill in all blanks. If the question does not pertain to you, indicate N/A. You must sign the Release of Information and the True and Complete Information statements. LVN to RN applicants must initial 30-unit Option statement and Route Selected.
You must have OFFICIAL SEALED HIGH SCHOOL AND COLLEGE TRANSCRIPTS (if other than LAHCC) and the Nursing Reading Comprehension Assessment Test results. If you have a GED or High School Equivalency from the United States you must attach the results of the test to your application. If you graduated from high school in a foreign country and are requesting high school equivalency based on a Professional Evaluation you must attach an official high school transcript from your country and an official copy of the evaluation. If you are seeking credit for courses taken outside the LACCD, which may be equivalent to required requisites, you must attach copy(s) of signed granted LAHC general petitions(s). If the other school will not give you official transcripts, then you are to have that school mail two (2) official transcripts to LAHC. One copy is to be sent to the Nursing Department and one to the Admissions Office. The copy sent to the Nursing Department should be addressed to the Health Science Division. Your official sealed transcripts from high school and any other colleges must arrive prior to deadline date.
Read and Sign the front side of one of the two blue (for RN) or pink (for LVN to RN) Application Forms. Complete the back sheet (Admission Eligibility Criteria) forms and submit attached to the application. You must list all dates, grades and where each class was taken. If a class is In Progress, write IP on the grade line. Keep the second copy for your records.
Nursing Application Deadline for Fall Admission is March 1st and for Spring Admission, September 30th. No application will be accepted after the deadlines. No late transcripts or scores will be accepted after the deadline unless requested by the Health Science Department.

THE APPLICATION MUST BE SUBMITTED BY MARCH 1ST FOR FALL SEMESTER AND SEPTEMBER 30TH FOR SPRING. READ, SIGN AND SUBMIT THIS SHEET WITH THE APPLICATION ALONG WITH ALL APPLICABLE INFORMATION.
BASIC ADMISSIONS REQUIREMENTS AND PREREQUISITES
1. Completion of all admission requirements of Los Angeles Harbor College.
2. Graduation from an accredited U.S. high school or equivalent – i.e. the General Education Development (GED) or equivalent
as verified by the Los Angeles Community College District – or graduation from an accredited U.S. college or university.
3. Passing the LAHC Math Competency Exam or satisfactory placement on the LAHC Math Placement Test with completion of a
college course in elementary Algebra (Math 123A/B, CT60 or ET49 or equivalent), or a higher math course with a grade of “C”
or better. High School Algebra and/or Nursing 302 DO NOT meet college graduation math competency requirement.
4. Completion of English 101 with a grade of “C” or better. Completion of Physiology 1, Anatomy 1 and Microbiology 1 or 20 with
a grade of “C” or better. Overall GPA in the three (3) science courses must be 2.5 or higher with a maximum of two (2) repeated courses in the three core science courses. Completion of Nursing 301A with a grade of “C” or higher. Achievement of a cumulative 2.5 GPA on all work completed at U.S. accredited colleges and universities is required for initial enrollment. A minimum GPA of 2.0 and a grade of “C” or higher in all nursing courses is required for continued enrollment in the program... Note: Students receiving credit for foreign college or university coursework will receive the equivalent of a grade of “C” for each course given such credit.
5. Psychology 1 and Psychology 41 are prerequisites for the 2nd semester and must be completed with a grade of “C” or higher.
6. Completion of one year of U.S. high school Chemistry with a lab or one semester of college chemistry with Lab (Chem. 65 or 55 or
higher) with a grade of “C” or higher. Chemistry is a prerequisite for Physiology and Microbiology.
7. All nursing applicants must take the L.A.H.C. Nursing Reading Assessment Test before the application deadline. Test dates and

through the Admissions and Counseling Office and the Assessment Center. The
score sheet must be attached to
your application.
8. Effective Fall 2007
students must pass a diagnostic assessment test (basic math, English, reading,
science) prior to program
admittance. Students who fail to achieve a passing score will be
required to complete additional pre-nursing course work before
admission will be granted. This requirement is due to recently
passed legislation.
All OFFICIAL sealed transcripts from high school and colleges other than LAHC must be attached to the application or the application will not be accepted. If the school will not mail official transcripts to the applicant, have them mailed to the Health Sciences Division of LAHC prior to the deadline dates. Make a note of this on your application. ALL TRANSCRIPTS FROM OTHER SCHOOLS MUST BE MAILED BY THE SCHOOL TO THE LAHC ADMISSIONS OFFICE. THE ADMISSIONS OFFICE DOES NOT ACCEPT WALKED IN TRANSCRIPTS.
ADDITIONAL ADMISSION INFORMATION
Applicants will be free from communicable disease, infection, psychological disorders and other conditions that would prevent successful performance of the nursing program’s education requirements. Upon acceptance to the program, applicant must submit the following no later than the first school day: A Cardiopulmonary Resuscitation (C.P.R.) Certificate good for one year (it must be a 1 and 2 person, adult, infant, child C.P.R. Airway Management course – B.L.S/C course); A current standard Red Cross First Aid and Personal Safety Certificate or E.M.T. certificate; The Health Record form must be completed and signed by a Physician/Health Service/Nurse Practitioner; verification of current Malpractice Professional Liability Insurance; a completed background check accompanied by the report and certificate.
RANKING OF APPLICANTS
When the number of applicants designated as being eligible for admission exceeds the number of available student positions in the program the ranking of the applicants is determined by the order of submission of the application for those who have completed all required prerequisites as stated above.
ADDITIONAL INFORMATION
Complete the back of this application information form. Indicate related courses completed, grades and courses in progress. Indicate any science classes repeated. Make sure all transcripts, petition, evaluations, etc. are attached. Make sure you have signed and dated the forms. All writing must be legible.
In order to graduate with an A.S. in Nursing, be eligible to take the licensure exam as a graduate and obtain the school pin, a student must complete, with a grade of “C” or better all required courses of the BRN approved curriculum as well as all course requirements of graduation Plan B, meet the math competency requirements and have a cumulative G.P.A. for all college coursework of 2.5 prior to admission to the program and a GPA of 2.0 or better thereafter. The student is responsible for the graduation check with a counselor, submitting the graduation petition, applying to the BRN for licensure and to the NCLEX-RN test service for a testing date in a timely manner, meeting all required deadlines.
It is the student’s responsibility to make certain that the Nursing Division has her/his current address as well as a current telephone number on file in the Nursing Division secretary’s office. Please contact a counselor in the Counseling Division (310) 233-4229 regarding any questions about the nursing program or the evaluation of transcripts. Group Counseling meetings are offered once a month in the Nursing Building. Call the Counseling Division or the Nursing Division for times and dates. Attendance is encouraged.
